What to Bring:

Birth Certificate

Social Security Card

Shot Record

Physical (within the last 2 years)

Completed Enrollment Form

Completed Nurse’s Packet

TWO utility bills showing a parent/guardian’s service address within the Barton School District (Examples: water, electric, gas)

Age Requirement:

Your child must be 5 years old on or before August 1, 2025 to enroll.
