Phillips Co. Health Unit is offering a Vaccination Clinic on September 7, 2022 at Barton. This is NOT the COVID-19 vaccine. This is for students who are behind on their regular vaccines.

\r\nThis form MUST be filled out and signed before any student can get a vaccine. If the form is not filled out and signed then the student will not get their shots.
